Print Affidavit/Agreement

Then continue with your registration including payment after acknowledgement.

Your registration will be complete when we receive your signed Affidavit/Agreement, along with a photocopy of the front of your valid photo ID. Please allow at least 48 hours processing time once everything has been submitted to receive your login instructions by email.

I will email/fax the required forms to complete the registration.
